Request for Quotation for 20 ERT Kit

Participatory Village Development Programme (PVDP) Sindh is a non-profit and non- governmental organization is implementing a project title “Humanitarian Assistance for flood affected households in Mirpurkhas Sindh (Continuation)” Project with the support of Diakonie Katastrophenhilfe (DKH). Sealed Quotations are invited from Vendors / Suppliers for the following items.

Key Requirements for your Quotation Submission:

• Provide the unit price for each item, lump-sum will not be considered.
• Quotation price should include all applicable taxes, including withholding tax and transportation expenses, loading, unloading, branding packages etc on company letterhead.
• Include the expiry date of your quotation, NTN number, phone number etc.
• Clearly state the payment terms in your quotation.
• Quotations should be submitted in a sealed envelope, also fill & sign the CONFLICT OF INTEREST DISCLOSURE FORM attached with this RFQ.
• Company profile with relevant experience including Client name, the volume of contract, and the location of services.
• 06 monthly bank statements (latest months).
• Payment details of the supplier must be clearly stated e.g., to whom the cheque should be addressed and if necessary.
• Delivery is to be made at 20 villages of Taluka Jhuddo District Mirpurkhas.
• Supplier must mention the delivery time.
• Supplier must submit the sample along with the sealed quotation.

Selection Criteria:

• The lowest price will not be the sole criterion; quality, delivery time, and previous experience will also be considered.
• The procurement committee reserves the right to change the quantities or cancel/reject any or all offers without assigning any reason.

Other:

• Suppliers must not be engaged in any corrupt, fraudulent, collusive or coercive practices including but not limited to applying/ bidding by multiple names / companies. If any bidder is found to be involved in such practices his/her bid may be rejected and the companies in question permanently blacklisted.
• PVDP reserves the right to change the quantities or reject any or all quotations without assigning any reason(s) thereof.
• PVDP will not be responsible for any damage and loss of goods during transportation.
